Case Management
Providing case management services to meet all social and medical needs for Senior Adults and People with functional and/or cognitive disabilities through the CCSP Waiver Program.

What is CCSP?
Georgia’s Community Care Services Program (CCSP) provides home and community-based services for elderly and / or disabled state residents who are functionally impaired and at risk of nursing home placement.
Services Available through the CCSP Waiver:
Personal Support Services (PSS)
A variety of supportive services provided in the individual’s home to assist with Activities of Daily Living (ADLs) and Instrumentals of Activities of Daily Living (IADLs).
Adult Day Health
Health, therapeutic, and social service activities provided in a group setting. Services include nursing care; occupational, speech, and physical therapies; personal care; dietary services; and social work services.
Alternative Living Services
Residential settings that provide 24-hour supervision and health-related support services to persons unable to remain in their homes.
Emergency Response Services
An in-home electronic support system which provides communication and monitoring to assist senior adults and individuals with disabilities during an emergency occurrence.
Home Delivered Meals
A variety of nutritious meals that cover all dietary needs.
Respite Care
Planned or emergency temporary care provided to caregivers.
Home-Delivered Services
Skilled services and medical social services provided by a certified, licensed home health agency.
